Job Overview
Community Physicians is seeking an experienced and compassionate APRN (FNP or Psych Certified) or Physician Assistant to join our growing team in a psychiatric setting. You will round at Skilled Nursing Facilities (SNFs), Assisted Living (AL), and Independent Living (IL) communities, working collaboratively with physicians to provide high-quality psychiatric care throughout the Central Illinois region. This role offers autonomy, flexible hours, and the chance to build lasting relationships with patients and facility staff.

Responsibilities:
- Conduct psychiatric evaluations and medication management
- Collaborate closely with supervising physicians
- Provide mental health services to geriatric and adult populations
- Maintain accurate clinical documentation
- Build professional relationships with facility staff and families
Qualifications:
- Licensed APRN (FNP or Psych) or PA in the state of Illinois
- Experience in psychiatric care strongly preferred
- Strong communication, organization, and work ethic
- Ability to work independently and collaboratively
- Valid driver’s license and reliable transportation
