Summary of Position
Calgary Foothills Primary Care Network's Extended Health Team (EHT) is comprised of health management nurses, pharmacists, a kinesiologist, occupational therapists, physical therapists, family physicians, nurse practitioners and mental health consultants. The team helps patients with complex chronic health conditions who struggle with psychosocial factors to self-manage chronic conditions, set achievable goals and access additional resources and support. Group and individual appointments or home visits are tailored to the patient’s needs.
Reporting to the Program Manager and Medical Lead, the Nurse Practitioner collaborates with family physicians, PCN interdisciplinary teams and community/external partners to provide patient-centered care for those with chronic disease and/or complex health conditions. The Nurse Practitioner will practice within their full scope of practice as outlined in the Standards of Practice by the College of Nurses of Alberta (CRNA).
Key Responsibilities
Key aspects of the position may include, but are not limited to:
- Perform advanced health assessments for complex adult patients in a biopsychosocial model
- Provide frailty assessment and cognitive screening to older adult patients
- Support EHT follow up of patients
- Synthesize assessment and past medical history to develop an individualized treatment plan with appropriate pharmacological and non-pharmacological recommendations
- Work in partnership with interdisciplinary team members and patients to facilitate patient behavior change and self-management
- Contribute to knowledge and clinical skills of EHT team members through one-on-one consultation, rounds and joint appointments
- Support the family physician in the medical home in caring for the patient through comprehensive written reports and telephone consultation
- Identify and implement opportunities for program improvement in conjunction with leadership
Knowledge, Skills, and Abilities
- Creates a safe environment for trusting client interactions
- Demonstrates advanced assessment skills in adults with chronic conditions and geriatrics
- Implements evidence based therapeutic interventions with the client to optimize health
- Able to facilitate group appointments and classes
- Demonstrates expertise in motivational interviewing and behavior change techniques
- Exhibits knowledge of chronic pain neuroscience
- Consults with physicians consistent with the role of the nurse practitioner
- Establishes collaborative relationships with health care providers on EHT, in the medical home and in the community
- Participates in the design and implementation of an evolving program
- Able to prioritize, manage time effectively and be flexible in a dynamic environment
- Strong computer skills are required
Qualifications
- Bachelor’s Degree in Nursing
- Master’s Degree in Advanced Nursing Practice
- Licensed with the College and Association of Registered Nurses of Alberta as a practicing Nurse Practitioner, with Family/All Ages (FAA) registratio
